Camp REGIStration

Registration forms will be accepted on a first-come, first-serve basis, since sessions fill up quickly.  Phone reservations are not accepted.

You can either print out copies of the medical forms below to include with the registration form, or we will email them to you after your registration form has been processed.   

Both parts of the Medical Form must be completed. Part A is to be completed by the parent/guardian. Part B requires proof that the child has had a physical after August 15, 2006. The copy of the physical must include the following:

      -->Proof of the child’s physical within the past two years that is clearly dated and signed by      the physician and specifically grants approval for the child to participate in sports camps. 

       -->A legible copy of immunization history must be included.

Incomplete medical forms will be returned and campers may forfeit their space in camp.

If you prefer, you can pay the full amount upfront.  The deposit is only to secure a position at the camp.  If you choose to only pay the deposit now, the full payment will need to be paid one month before the first week of camp.  We will confirm your deposit via email as soon your application is processed.  The email will also notify you of your balance.

If a week you are looking for is closed you may call the Swim Clinic Office to be placed on the waiting list.  Please fill out the form, but you do not have to send a deposit unless space is made available for your child.

REGIStration Fees

The Regis College Swim Clinic offers two options for the clinic and three discounts to those who are eligible.

Regular Session–  ($375) The regular 8:30 am to 3:30 pm clinic session.

Extended Day–  ($75)  Although the swimmers are given another chance to swim, extended day session is less structured.  It is primarily offered as a convenience for parents who can not pick up their children at 3:30 pm.  Extended Day runs until 5:30 pm.  Extended Day will no longer be offered for Week 1.

Early Application – Mail your registration form before June 1st and automatically receive a $25 discount.

Family Discount– Parents who wish to sign up more than one of their children will be offered a discount of $25 for each of two or more children.

Regis Faculty Discount– A discount of $25 is offered to children of Regis College Faculty.

 A non-refundable deposit of $75.00 per child per week is required with the Registration Form.

Only needed if anyone but the two guardians listed on the registration form is going to pick up your child.  These will also be availible every day of camp durring drop-off.
